Extended Care service (sometimes known as Long-Term Care) at George C. Grape Community Hospital provides rehabilitative, restorative, and ongoing skilled nursing care to patients or residents in need of assistance with activities of daily living.

We offer a menu of services. A person may need only one or a few kinds of services. Or, several kinds may be needed over the course of a person's older years. These needs may change over time.

When you or a loved one find it difficult to complete daily tasks that once seemed easy, George C. Grape Community Hospital's Extended Care Program can give you and a loved one peace of mind with exceptional, compassionate care.

  • During Business Hours
  • 8:00 am - 5:00 pm
  • Check-in at Main Entrance

  • After Hours | Weekends

Emergency Room Hours:


  • 24 hours a day
  • 7 days a week
  • 365 days a year

Specialty Clinic Hours:


  • Monday - Friday
  • 7:00 am - 3:30 pm

Saturday Clinic Hours:


  • 9:00 am - 12:00 noon
