Home Health Care Clinic Coordinator - Home Health Care
The SWIHH Clinic Coordinator directs clinical care of home health clients and supervises RN Case Managers.

Maintains high level of clinical competency and meets all requirements of RN Case Manager.
Directs nursing and home healthcare as prescribed by the attending physician. Reviews all plans of treatments for accuracy, appropriateness and completeness.
Evaluates and regularly reevaluates the therapy and nursing needs of each patient.
Supervises and trains other nursing services personnel. Provides orientation to new personnel.
Participates in the Agency’s information management and performance improvement programs
Coordinates all patient services by reviewing plans of treatment, assuring case conferences are conducted, reviewing clinical and progress notes, periodic patient evaluations and determining frequency and/or staffing patterns.
Maintains a working knowledge of current Medicare, Medicaid and third-party payer requirements for home care reimbursement.

Qualifications:

Current Iowa RN License
Two years home care management
Excellent clinical skills
Thorough knowledge and understanding of home care regulatory requirements.
Job Type: Full-time, Mon-Fri 0800-1630 on-call rotation
