Utilization Review & Chronic Care Management (CCM) Nurse

George C. Grape Community Hospital & Clinics is seeking a full-time Utilization Review & Chronic Care Management (CCM) Nurse. This unique role bridges hospital inpatient care and outpatient clinic coordination, making a direct impact on patient outcomes.

About the Role
In this position, you’ll split your time between:
• Hospital Utilization Review – Conduct daily face-to-face rounds with providers to review care plans, coordinate timely discharges, and ensure documentation supports medical necessity.
• Clinic Chronic Care Management – Work with patients who have multiple chronic conditions, creating care plans, providing education, and ensuring they receive the right care at the right time.

Qualifications
• RN Iowa.
• At least 2 years of nursing experience (hospital, clinic, or care coordination).
• Strong communication skills for daily face-to-face provider interactions.
• Organized, detail-oriented, and comfortable with electronic health records.
• Self-motivated, dependable, and driven to take initiative.
• Passion for helping others and making a positive impact in patients' lives.
