Community Health Needs Assessment - George C. Grape Community Hospital

George C. Grape Community Hospital began working on their Community Health Needs Assessment. By the end of September 2013, George C. Grape Community Hospital in collaboration with community members from around the county will be identifying areas to focus on to better the health of our community over the next three years.
You can help play an important role in deciding what the areas of focus should be. Community members will soon be receiving a survey in their mailbox; this survey will help George C. Grape Community Hospital, Southwest Iowa Home Health and Fremont County Public Health officials analyze community needs, enhance local health care services and improve overall community health.
Residents will be receiving the survey via the "Grab the Gold" monthly hospital newsletter the first part of April which can be filled out and returned to your local city office. Residents with access to the Internet may quickly complete the survey on-line by accessing the George C. Grape Community Hospital Website at www.grapehospital.com. There will be a link for you to follow.

"The goal of this initiative is to help us identify and address the present and future needs of healthcare in the best way possible."
The results of the assessment and a health implementation plan will be available for community review by September 2013.
