Community Health Needs Assessment




Every three years, the Hospital facilitates a collaborative community health needs assessment (CHNA) to meet IRS regulatory requirements and to identify and prioritize the health needs of Fremont County residents.

Healthcare professionals and organizations in Fremont County have a long history of responding to health needs in the communities they serve. This commitment extends beyond traditional hospital and clinical care to community health programs that provide services to the region's most vulnerable populations.

Community health programs are based on an assessment of needs identified through hospital data, community input, and major trends (national, state, and local statistics). We depend on the cumulative results and feedback from these community engagements:

  • county residents
  • community-based organizations
  • social service providers
  • health care leaders
  • online survey for residents and stakeholders
